Please read fully and carefully before booking.
We have policies in place to ensure a comfortable, safe, and respectful environment for both our patients and staff. We kindly ask that all individuals treat our team with courtesy and respect as these policies are upheld.
If you are not comfortable adhering to our policies, we respectfully ask that you refrain from proceeding with booking.

No extra guests are to accompany you during the appointment unless they are getting serviced as well. – NO EXPECTATIONS!!!!!!!!!
We at Unique Esthetics, are very dedicated to providing you with the best care possible. For that reason, your appointments is reserved specifically for you with plenty of time to discuss concerns, ask questions or get an amazing treatment with one of our highly trained providers!
LATE POLICY
We will give each client a 10 min grace period after that you will be changed an additional 25$.
If you are more than 15 minutes late your appointment you may be cancelled, and you will need to reschedule.
ADDITIONAL INFORMATION
Once you book we will reach out to you to confirm your appointment, we will request a picture of your lips to be sent prior if getting lip filler.
If getting Tox we will ask you for pictures of you making a mean face, smiling and raising brows.
We will set up your appointment with NP.
Special pricing you give consent for your pictures to be used for educational purposes and or marketing. You have the right to refuse photo consent and will be responsible for total cost. We can block/cover things per your request.
CANCELLATION POLICY
Same-day cancellations or no-call, no-shows will require a non-refundable $50 deposit at the time of your next booking.
• Two (2) cancellations will require a 50% service deposit at the time of booking.
• Three (3) cancellations will result in inability to book with Unique Esthetics in the future.
• Two (2) no-call, no-shows will result in inability to book with Unique Esthetics in the future.
Appointments scheduled with our RN, NP, or DNP providers require a 96-hour cancellation notice. Failure to provide proper notice will result in forfeiture of your deposit and a $50 cancellation fee, which must be paid prior to rebooking.
Model Day appointments require a minimum of 72-hour cancellation notice. Failure to cancel within this timeframe will result in loss of your deposit.
For all other appointments, we kindly request at least 72-hour notice for cancellations.

VIRTUAL ASSESSMENT (GOOD FAITH EXAM)
Prior to receiving any aesthetic service, a Virtual Assessment—also known as a Good Faith Exam (GFE)—is required. This is a medical evaluation performed by a licensed healthcare provider to determine if a treatment is safe and appropriate for you.
During your assessment, our Nurse Practitioner will review your medical history, allergies, and any previous treatments. She will also walk you through the procedure step-by-step, discuss potential side effects, and provide detailed aftercare instructions. Our goal is to ensure you feel fully informed, confident, and prepared prior to your service.
During botulinum toxin (Botox) appointments, our Nurse Practitioners carefully assess your facial anatomy and muscle movement to determine the appropriate number of units needed for optimal, natural-looking results. Treatment plans may vary from patient to patient and between visits.
To ensure the best outcome, we recommend scheduling a follow-up appointment within 10–14 days to assess your results and determine if any adjustments are needed.
If additional units are required at your follow-up appointment, they will be applied at an additional cost.
You will receive a link via text to complete your Virtual Assessment with our highly trained NP, Jenna, prior to your appointment. We aim to have this completed at least 24 hours in advance. If your appointment is with Jenna, a virtual assessment is not required. Jenna will complete your full evaluation and treatment plan at the time of your appointment.

Failure to attend your scheduled Good Faith Exam will result in a $30 rebooking fee. Our team makes every effort to coordinate a time that works best for you, and we kindly ask that you respect this appointment.

FIRST-TIME CLIENTS
Effective July 2, 2025, all first-time clients are required to complete a two-week follow-up appointment.
CONSENT FORMS
All services performed by a Registered Nurse require completed consent forms and questionnaires. These will be sent to your email via Aesthetic Record.
All forms must be completed prior to your appointment. If you have any questions, please contact the spa.
If you are unable to complete your forms in advance, please arrive 15 minutes early. Failure to complete forms prior to arrival or arriving late may result in your appointment being rescheduled and a $50 fee will apply.
DEPOSITS
A deposit is required to book all appointments.
No deposit = No appointment.
Deposits are non-refundable for all services, shadowing, and classes. However, you may reschedule classes or shadowing.
PAYMENTS
We accept cash, FSA/HSA, and all major credit cards.
We also offer payment plans through Klarna, AfterPay, Shop and AffirmCherry and CareCredit.
A 10% service fee will be applied to all Cherry payment plans.
No coupons or discounts may be combined with special pricing.
Gift cards are valid for one year from the date of purchase.
By booking an appointment, you agree to all policies listed.
REFUNDS: SERVICES
We do not offer refunds on services rendered. All service sales are final.
Unused service value (remaining balance from a package) may be applied toward other services at Unique Esthetics.
All injectable treatments are final sale. No refunds or credits will be issued once treatment has been completed.
REFUNDS: PRODUCTS
Unopened products may be returned within 10 days of purchase with a receipt.
If an adverse reaction occurs, store credit may be issued following consultation with your provider. Products must be returned within 30 days with proof of purchase.
PRODUCT SHIPPING:
We offer shipping for medical-grade skincare. Shipping starts at $12 and may increase based on order size or weight.
Additional fees apply for signature confirmation or insurance.
Once an order has shipped, Unique Esthetics is not responsible for lost, stolen, or damaged packages. No refunds or replacements will be issued.
If your package is lost or damaged, please contact the United States Postal Service to file a claim.
REFFERAL PROGRAM:
Refer one (1) client: Receive 10% off your next service.
Refer two (2) clients: Receive 20% off your next service.
Refer three (3) or more clients: Receive a complimentary service.
SEAT FEE:
All services performed by RN, NP, or DNP providers require a $100 seat fee.
This also applies to raffle winnings.
HOME SERVICES:
Home services are available for a $50 flat fee plus mileage (subject to change).
All pets must be secured prior to the appointment.
Clients must ensure a safe, respectful, and non-violent environment for staff.
UNDER 21 YEARS OF AGE:
A parent or legal guardian must sign a consent-to-treat form for all patients under 21.
A copy of the parent/guardian ID is required for verification.
Lip filler is FDA-approved for patients 21 and older. Patients 18–20 may be eligible with parental consent.
Virtual consent with a parent or legal guardian is available if needed.
