Bring a Valid Photo ID
A government-issued photo ID is required for all appointments. We are unable to proceed without proper identification.
Complete Required Forms
Please complete all intake and consent forms prior to your visit. If you experience any technical issues, our team is happy to assist you. Please text 234-269-0600.
Arrive 10–15 Minutes Early
Arriving early allows time to complete any remaining paperwork and gives you the opportunity to ask questions or discuss any concerns before your service begins.
Flexible Payment Options

Virtual Assessment
Good Faith Exam – Pre-Aesthetic Service Information
This will be done virtually on your phone.
Before receiving any aesthetic services, a Good Faith Exam is required. This is a medical evaluation performed by a licensed healthcare provider to ensure that the treatment is safe and appropriate for you. It may also be referred to as a pre-authorization exam, clearance exam, initial exam, or physical exam.
Our Nurse Practitioner (NP), Jenna, will:

Review your medical history, allergies, and any previous treatments

Explain the procedure step by step

Discuss potential side effects and provide thorough aftercare instructions
Our goal is to make sure you feel fully informed, confident, and prepared—with no unanswered questions—before your appointment. If you have booked with Jenna a virtual assessment is note needed
Your Good Faith Exam
This virtual exam will be scheduled the day before your appointment.
We’ll reach out to coordinate a time that works best for your schedule.
If your appointment is with Jenna, a virtual assessment is not required. Jenna will complete your full evaluation and treatment plan at the time of your appointment.
If you are receiving neurotoxins (e.g., Botox), please send clear photos of yourself:

If you have previously had lip filler, please send a current photo of your lips.
