Virtual Assessment
Good Faith Exam – Pre-Aesthetic Service Information
This will be done virtually on your phone.
Before receiving any aesthetic services, a Good Faith Exam is required. This is a medical evaluation performed by a licensed healthcare provider to ensure that the treatment is safe and appropriate for you. It may also be referred to as a pre-authorization exam, clearance exam, initial exam, or physical exam.
Our Nurse Practitioner (NP), Jenna, will:

Review your medical history, allergies, and any previous treatments

Explain the procedure step by step

Discuss potential side effects and provide thorough aftercare instructions
Our goal is to make sure you feel fully informed, confident, and prepared—with no unanswered questions—before your appointment.
Your Good Faith Exam
This virtual exam will be scheduled the day before your appointment.
We’ll reach out to coordinate a time that works best for your schedule.
What We Need From You:
If you are receiving neurotoxins (e.g., Botox), please send clear photos of yourself:

If you have previously had lip filler, please send a current photo of your lips.

Consent Forms & Questionnaires – Required for All Services
All services performed by our Registered Nurse and select treatments by our Licensed Esthetician require completed consent forms and questionnaires. These forms will be sent to your email via Aesthetic Record.
