The Perfect Derma The Perfect Derma™ Peel is a medical grade medium-depth peel featuring the powerful antioxidant, Glutathione. This antioxidant brightens dull-looking skin, temporarily delays visible skin aging, and prevents the onset of wrinkles. The Perfect Derma™ Peel is suitable for all skin types and ethnicities, requires very little downtime and is almost completely pain-free. This is a 7 day peel that brightens and clarifies the skin. The peel comes with a post peel kit to protect and soothe your skin during the peeling process. For optimal results, a series of 2-4 treatments every 4 weeks is recommended. WHO IS IT FOR? The Perfect Derma Peel is for all skin types and fitzpatricks. It's perfect for those with: • Hyperpigmentation • Melasma • Visible sun damage • Uneven skin tone or texture • Dull skin • Enlarged pores • Acne-prone skin • Acne scarring The Perfect Derma™ Peel is a medium-depth chemical peel that removes the top layers of the skin for a smoother, brighter complexion. It combines Glutathione and effective acids like TCA, retinoic acid, salicylic acid and kojic acid to improve overall skin clarity, reduce hyperpigmentation and scarring, and stimulate the production of collagen. POST TREATMENT KIT This service also includes a Post Treatment Take Home Kit. Kit Contents: • 2 Post Peel Towelettes • 1 Post Peel Moisturizer (1oz.) • 1 Mineral Perfection SPF 30 (1oz.) • Post peel instructions. The Take Home Kit is designed to ensure an easy-peel process for optimal results.

VIRTUAL ASSESSMENT Prior to any Aesthetic Services A good faith exam is a medical evaluation that a licensed healthcare provider performs to determine if a procedure or treatment is appropriate for a patient. It's also known as a pre-authorization exam, clearance exam, initial exam, or physical exam. Our NP will go over medical history, allergies, any previous services. She will go over how procedure is done step by step. She will go over any side effects and aftercare instructions. We want you to ensure that you are educated thoroughly prior to your service. You will receive a link via text, for your Virtual Assessment to be completed with one of our Highly Trained NP Jenna prior to your appointment. We typically aim to get it completed 24hours prior to your appointment. Virtual Assessment/ GFE is needed prior to service. Please log-in at least 10 minutes early and make sure you have access to good internet. If you are having problems with the link, please reach out to our Spa Assistant Christine 330-906-9763. If you miss you GFE with our NP there will be a 30$ rebooking. We personally reach out to you to ensure that we have the perfect time selected by YOU. FIRST TIME CLIENTS Effective July 2, 2025 all first time clients are REQUIRED TO DO A TWO WEEK FOLLOW UP APPOINTMENT. CONSENT FORMS All services performed by Registered Nurse, REQUIRE you to fill out consent forms and questionnaires, they will be sent to your email from Aesthetic Records. All consent forms MUST be completed prior to appointments. If you have question, please contact Christine our Spa. Please have your forms filled out prior to your scheduled appointment time. If you are unable to fill out forms prior to your scheduled appointment please arrive 15 minutes early. If forms are not filled prior and you do not arrive to the office 15 minutes prior to your appointment. Your appointment may need to be rescheduled and a fee of $50 will apply.
